Photo release for a child
I am the parent or legal guardian of the child named above. I give Pocket Creek Child Care permission to use photographs and short videos of my child for the purposes I have marked “Yes” above, and for no others.
What it covers. Pictures taken at the center, in the yard or on a field trip. Pocket Creek will not publish my child’s last name, and will not publish my child’s first name next to a photo.
Where it may appear. Only in the places I marked: this website, the center’s social media accounts, and printed materials such as flyers, the lobby board and the newsletter.
Taking it back. I can withdraw this permission at any time by telling the director in writing. Pocket Creek will stop using my child’s photos going forward and will take down photos it controls within a reasonable time. It can’t take back printed materials that have already been handed out.
If I choose no. My child will still take part in every activity. Teachers will keep my child out of photos used for these purposes.
I am 18 or older, and I have read this release, understand it and sign it of my own free will.
